Founding, standard, and family membership options
Founding tiers are limited to the first 50 members and begin with a 3-month initial commitment. Founding member rates are locked in for as long as your membership stays active, a thank-you for being part of Somaform from the beginning.
Founding Adult Membership
$115 / month
Available to the first 50 founding members with an initial 3-month commitment.
Standard Adult Membership
$135 / month
Enhanced access membership for established members after founding enrollment.
Founding Family Cap
$245 / month
For two adults and dependent children under 18, limited to founding enrollment.
Standard Family Cap
$275 / month
Family access model after founding membership enrollment closes.

Family cap options for coordinated preventive care

Select Your Membership Type
Choose from an Individual Plan or Family Plan. The first 50 enrollees will automatically become Founder Members, which is your tier for the duration of your membership.
Submit Payment
Your membership benefits start as soon as you submit your first payment. It's easy to pay only with with a credit card in minutes.
Book Your First Appointment
For now, appointments are available via telehealth, with in-person appointments beginning in fall 2026. Founding members will receive first access to in-person booking at our clinic in Tewksbury.
